Thanks for taking the time to fill out our survey!

Your responses to this survey will be posed on our website on the "Faces Of Union" page.

Be assured we will not post addresses or telephone numbers.

If at anytime you would like your information removed or changed, just contact Kathy Gardiner at webmaster@unionumc.org and this will be handled within 24 hours.

Photographs:

We plan to use the photograph taken by Olan Mills in 2006 unless you have a picture you would prefer to use with your survey information.

If you would prefer an updated photo, mark the appropriate button on the survey. Kathy Gardiner will contact you to set up a time to take a photo for use on Faces Of Union.

Multiple Family Members?:

You can fill in your answers for your entire family on one survey or submit one per family member and we'll put them together for final posting.
